A few things to know before you arrive.
Please arrive early
Arrive 30 minutes before your scheduled appointment to complete check-in.
Prepare for skin testing
Stop antihistamines at least 48 hours before an allergy evaluation. Atarax and Hydroxyzine should be stopped one week before your appointment.
Cancellation policy
Please give 24 hours’ notice to avoid a $25 cancellation fee. Opening your time may help another patient be seen sooner.

What to bring
Bring what helps us understand the full picture.
- Your insurance card and referral authorization, if you have an HMO
- A list of medications, including dose and frequency
- All completed patient forms
- Recent test results related to your condition
- Foods you suspect you are allergic to
- A nasal sample in a ziplock bag
Referrals & authorizations
Please confirm your current authorization.
Many HMO plans require a current referral and/or procedure authorization. If your insurance requires it, please make sure we have a current referral from your primary care physician before a visit and current authorization before procedures.
